Families First Coronavirus Response Act

The Families First Coronavirus Response Act (FFCRA or Act) requires certain employers to provide their employees with paid sick leave and expanded family and medical leave for specified reasons related to COVID-19. These provisions will apply from April 1, 2020 through December 31, 2020.

Health and Welfare Wrap Summary Plan Description (SPD)

Rambus maintains a Plan for the exclusive benet of its benefits eligible employees and their eligible family members. The Plan provides health and welfare benefits through the benet programs listed in within the Wrap Summary Plan Description (SPD) document attached below. Each of these benet programs is summarized in a certificate of insurance booklet issued by an insurance company.

ERISA, the Employee Retirement Income Security Act, is a Federal law that sets minimum compliance standards for Employers on their health and welfare benefit plans to provide protection for employees and their dependents enrolled in these plans. The Wrap SPD and its attachments, combined with the policies and benet plan documents listed within it, establishes the plan required by ERISA § 402 and constitutes the Summary Plan Description as required by ERISA § 102.

HIPAA Privacy Notice – Coming Soon!

Rambus understands that information about you and your health is personal and we are committed to protecting this information. Rambus maintains a Notice of Privacy Practices that explains how we may disclose your health information. The Notice of Privacy Practices also describes your rights and our obligations regarding the use and disclosure of this information. You have the right to request a paper or electronic copy of the Notice of Privacy Practices by contacting your Human Resources or Benefits Department.

 

Notice of Exchange

The Notice of Exchange explains that you may be eligible for discounted health coverage if you purchase health insurance through the Marketplace instead of enrolling in the plan(s) offered through us. We are required under the federal health care law (the Patient Protection and Affordable Care Act or PPACA) to provide you this notice. Please note that because our health plan is considered “affordable” and provides “minimum value” coverage as defined under PPACA, you may still be eligible for Marketplace coverage but this Marketplace coverage generally will not be subsidized by the federal government.

  • Availability of Privacy Practices Notice
  • Notice of Choice Providers
  • Premium Assistance Under Medicaid and the Children’s Health Insurance Program (CHIP)
